When should a coach refer out instead of coaching on?

Refer out whenever a client situation needs diagnosis, treatment or clinical management rather than coaching: undiagnosed or worsening pain, disordered-eating signs, a diet to treat a disease, chest pain or an uncontrolled condition, and significant mental-health concerns are refer-now. What is a coach's scope of practice?

Scope of practice is the line between what a fitness or nutrition coach is qualified to do and what belongs to a licensed professional. In your lane: designing training and general nutrition and habits for a healthy adult, coaching behaviour change, and progressing a program. Outside it: diagnosing pain or a condition, treating or rehabilitating an injury, prescribing a therapeutic diet to manage a disease, managing medication, and handling disordered eating or significant mental-health issues. Those are physio, doctor, registered dietitian and therapist territory. When a client situation crosses the line, the professional move is to refer out, keep coaching what is in your lane, and document it - not to push on and hope.

When should a coach refer a client to a doctor, physio or dietitian?

Refer whenever the situation needs diagnosis, treatment or clinical management rather than coaching. Red flags include undiagnosed or worsening pain, numbness or radiating pain, an eating disorder or its warning signs, a request for a diet to treat a disease, chest pain or an uncontrolled medical condition, and significant mental-health concerns. Proceed only with written medical clearance for pregnancy and postpartum, clients on GLP-1 or other weight-loss medication, older adults with health issues, and anyone with a controlled chronic condition - and coordinate with their clinician. When in doubt, refer out: it protects the client and it protects you.

Can a personal trainer coach a pregnant client or one on Ozempic?

With care and clearance, often yes for the training and habit side, but it is amber, not green. For a pregnant or postpartum client, get written clearance from their doctor or midwife and ideally hold a pre- and post-natal qualification; work within the clearance and refer anything clinical back. For a client on a GLP-1 medication like Ozempic or Wegovy, the prescribing is the clinician's - your lane is training, protein and muscle preservation, and habits, coordinated with their prescriber, never advising on the drug or the dose.
