When a client's shoulder flares on pressing, swap overhead and flat barbell presses for a neutral-grip dumbbell press, landmine press or floor press, and swap wide-grip work for cable or machine variations; when a knee complains on squats, swap to a goblet box squat, leg press or split squat through a pain-free range. Drop loaded end-range for the painful pattern, rebuild load over two to three pain-free weeks, and keep progressing everything that does not hurt. This is training around a niggle, not rehab: refer the client out if pain radiates, numbs, wakes them at night, or has not settled in ten days.

A good swap holds the training stimulus - the same movement pattern and primary muscle - while removing the position or range that provokes the joint. A barbell bench becomes a neutral-grip dumbbell or floor press: still horizontal pressing, still chest and triceps, but the shoulder keeps out of the painful bottom-range stretch. The client keeps training, keeps adhering, and keeps their momentum instead of sitting out a block. Stay in your lane: this is exercise selection, not diagnosis. How do I know if I should refer a client out instead of swapping exercises?
Refer out, before touching the program, if any red flag is present: night pain that wakes them, numbness, pins and needles or radiating pain down a limb, visible swelling or deformity, pain after a specific trauma, or an ache that is not improving after seven to ten days of sensible modification. Those are clinician territory. Swapping exercises is only appropriate for a minor, mechanical, non-radiating niggle in an otherwise healthy client who can still move the joint pain-free through part of its range.
