Kenneth Cooper built the test for large-group field assessment, so it needs almost nothing: a measured flat course, a stopwatch, and an honest maximal 12-minute effort. That last part is the catch - going out too hard and fading, or cruising with something left in the tank, both pull the estimate away from the client's real ceiling, which is why a pacing rehearsal before the real test is worth the session. The Cooper 12-minute run is the classic: run as far as possible in exactly 12 minutes, then apply VO2max = (distance in meters - 504.9) / 44.73. It rewards honest pacing - going out too hard or cruising both skew the result. The 1.5-mile run flips the format (fixed distance, measured time) and uses VO2max = 3.5 + 483 / time in minutes; a 12:30 finish is 3.5 + 483 / 12.5 = 42.1 ml/kg/min. Both need a maximal effort, so they suit clients cleared for hard running.
The resting-heart-rate estimate, VO2max = 15 x (HRmax / HRrest), needs no test at all - just a morning resting pulse and a max heart rate (measured, or estimated as 220 minus age). It is the roughest of the three and best used as a first screen for clients who cannot yet run all-out. The training that actually raises VO2 max.
VO2 max responds to two levers. The first is aerobic base: most weekly cardio volume at an easy, conversational intensity, which grows the heart's stroke volume and the muscles' ability to use oxygen. The second is time near max: 1 to 2 weekly interval sessions that hold the client at 90 to 95% of max heart rate - the well-studied 4 x 4-minute protocol with 3-minute easy recoveries is the workhorse. Expectations should scale with training age. Untrained clients often improve 10 to 20% inside 8 to 12 weeks; well-trained athletes fight for low single digits. Bodyweight matters too - because the number is per kg, fat loss alone raises relative VO2 max even before the engine improves. Re-test every 6 to 12 weeks with the same field test under similar conditions, and read the trend, not any single result.

Endurance athletes commonly test in the 60s and 70s. Treat every band as a guide, not a diagnosis.
